Death Penalty -

Death Penalty is a simple modification which gives you a few negative attributes upon respawning:

• You are left with 8/20 hunger points (at 6/20 you are unable to sprint!)
• You are giving slowness 1 for 15 seconds
• You are giving blindness 1 for 2 seconds

The creation is also very easily modified; just extend the command block chain to add more effects.  

More Information - 

The creation spawns on the west of the command block, and is 3 wide, 5 deep, and 3 tall.
