Many serious accidents have occurred because the machine was running when it was supposed to shut off to prevent maintenance or repairs. These accidents are often the result of stored energy that has not been properly released. Lock-out tag-out (LOTO) systems can help avoid these accidents by making sure that machines are off until the person who is authorized releases them.
The LOTO system is comprised of a number of locks tags, locks, and other equipment that are used to isolate, secure or shut off the device from electrical power. All "affected", "authorized" and "qualified" personnel should be instructed on the use of these devices. These devices include, but are not only restricted to locks, chains and padlocks. Each device is clearly identified and labeled with the date and time of application along with the name of the authorized individual, and the reason behind the lockout/tag-out (repair maintenance, repair, etc.). It must be clearly stated that these devices are not to be removed until the LOTO procedure is completed.
