Titration

A thorough assessment of your ADHD symptoms will be conducted with a highly experienced consultant psychiatrist. This includes a complete medical history, as well as an assessment using the standard ADHD questionnaires and rating scales.

After your evaluation, you will be informed if medication is considered to be beneficial for your situation. This will be discussed at your follow-up appointment (which could be in person, via video link or by phone). Your physician will explain to you what medication options are available and if you decide to start on medication we will recommend titrating the dose to maximize the benefits/side effects balance for you.

This is a process which could take a few weeks and requires adjusting your child's dosage gradually until the proper balance is achieved. It is not unusual to need to titrate several times before finding the one that is best for your child's ADHD symptoms. Parents must be involved in this titration process and keep an watchful eye on their child's behavior, so that they can let the doctor know whether the medication is helping or causing adverse effects.

If you decide to return to NHS treatment and have decided to return to NHS care, your medication will be given as a private prescription which you'll have to pay for at the pharmacy (this is not included in your consultation fee). You will be required to attend six monthly appointments to monitor your medication. This includes blood pressure and pulse readings. Annual reviews are also required.

Follow-up appointments

Once the evaluation has been completed you will be able to book follow up appointments with your psychiatrist, if necessary. The appointment is usually done through a zoom video call. However, in certain instances, it could be possible for an audio-only appointment to be conducted (again with an additional cost).

We will then discuss treatment options and decide on the most appropriate, customized ADHD medication adjustment. This will include a review of your medication dose and a discussion of any possible negative side effects you may be experiencing. The cost of these titration appointments is PS150 for face-to face sessions and PS60 for online or phone video consultations. The pharmacy will be charged for any cost of medication.

The psychiatrist will ask you questions about your family history and the impact of your ADHD symptoms on your life, and any mental health issues you may be experiencing. This is because ADHD can be a genetic disorder and it is important to look into any family history that may indicate whether you are at risk of developing the disorder.

It is important to be honest and open with your doctor during this assessment, as the information they get will be used to determine if your ADHD is in line with the diagnostic criteria. This is an essential part of the process, as failing to do so could lead to an inaccurate diagnosis, and thus an incorrect prescribing of medication.

If you've decided that you'd like to be treated for your ADHD, we will then schedule a follow-up appointment with one of our psychiatrists who specialise in this field. Check if your GP accepts a Shared Care Agreement, which means that you will be referred to them once you leave the clinic for prescriptions for medication.

Medication

The medication process is an essential part of the journey for many people looking for a private ADHD diagnosis. It isn't easy to obtain prescriptions from psychiatrists on the NHS. NHS waiting lists can be extremely long, and GPs may be reluctant to refer their patients to specialist services.

Asking your GP to refer through Right to Choose is one way to avoid this (see psychiatry - UK and ADHD 360). You can select a provider who offers a private assessment service and titration. This will help you save time and be more convenient, particularly if you have a busy work schedule.

You will need to fill out forms that are different for each provider, but the most common ones are self-reporting forms and medical histories dating to childhood and adulthood. These forms will help your clinician determine if there are any circumstances that indicate you shouldn't take ADHD medication, even if they have been prescribed.

If you are being titrated on medication, this will usually consist of stimulants and non-stimulant medications. These will be carefully trialled and monitored to determine the best combination for you, taking into consideration any current conditions you suffer from. The titration procedure can be lengthy and may require several extra appointments before you are stable.

The frequency of your follow-up appointments is contingent on the type and amount of medication you're taking. They can be monthly or quarterly. Your clinician will check whether you are safe with the medication, note any side effects that you've experienced and make any needed adjustments to ensure that you remain on a safe dose. You will be required to attend an annual review as well as a routine health exam that includes pulse, blood pressure and weight measurements.

For some individuals it could be that they are content with their medication and don't need any further discussion on their titration. For those who require further discussions regarding the titration process and treatment plan, it is possible that they will be passed back to the same private clinic that performed their initial assessment. In this case the titration is included in the consultation cost. The medication will then be provided through the private prescription. It could cost more than an NHS prescription because the pharmacy will charge for the medication.
