Titration

A highly experienced psychiatrist consultant will conduct a thorough evaluation of your ADHD. This will include an examination of your medical history as well as an assessment using the common ADHD rating scales and questionnaires.

After your examination, you will be advised that medication may be beneficial in your case. This will be discussed at your follow-up appointment (which may be scheduled in person, via video link or over the phone). Your doctor will go over the various options for medications available. If you decide to begin medication, we will help you the option of titrating your dose to maximize the benefits/side effects balance for you.

This is a process which could take a few weeks and involves adjusting the dosage of your child's medication slowly until the right balance is reached. It is not uncommon to have to adjust up to three different medicines until you discover the one that best suits your child's ADHD. It's important for parents to take part in this process of titration and keep a close eye on their child's behavior, so that they can inform the doctor whether the medication is working or causing adverse effects.

If titrating medication decide to go back to NHS treatment you will receive a private prescription for your medication. You will have to pay the pharmacy. This is not included in the consultation fee. You will need to keep up with your appointments every six months to monitor your medications. This includes the measurement of pulse and blood pressure. Annual exams are also required.

Follow-up appointments

After the assessment is completed, you will be able to book appointment with the psychiatrist as necessary. The appointment will usually be conducted via zoom video call. However, in certain instances, it could be possible for an audio-only appointment to be conducted (again with an additional cost).

At this point we will discuss any treatment options and come up with a unique ADHD medication titration strategy. This will include a discussion and review of your dosage of medication, as well as any side effects you may be experiencing. These titration sessions cost PS150 for face-toface sessions and PS60 via video or telephone consultation. The pharmacy will charge you for any charges for medication.


The psychiatrist will ask you questions regarding your family history as well as the effects of your ADHD symptoms on your life and any relationship problems or mental health issues you might be suffering from. This is due to the fact that ADHD can be a genetic condition and it is crucial to examine any family history that could suggest that you are at risk of developing the disorder.

This can be scheduled at the time of your diagnosis if you want to start treatment immediately, or within two weeks if want to think about the implications of your diagnosis and the alternatives that are available to you.

We suggest that if you are unsure about your preferences, talk with your GP prior to booking your private appointment. Certain providers have different policies on whether or not they require the submission of a GP letter. We also advise that you check whether your GP accepts a "Shared Care Agreement which means that once you are discharged from the clinic you will be returned to them for the prescription for the medication.

Medication

For many people who seek an official diagnosis of ADHD medication is a key part of their journey. The process of getting prescriptions from an NHS psychiatrist can be a hassle and lengthy. NHS waiting lists are notoriously long and GPs are often reluctant to refer patients on to specialist services.

Asking your GP to refer you to Right to Choose is one way to avoid this (see psychiatry - UK and ADHD 360). This means that you can select one that provides a private assessment and titration. This will cut down your waiting time significantly and is more convenient if you are working and have a full schedule.

Once you have been identified as a candidate for this treatment the doctor you choose will examine you and decide whether or not you're required to be monitored for a dose of medication. You'll need to complete forms that differ by provider but the most frequent ones are self-reporting forms and medical histories going to your childhood and into adulthood. These forms will assist your doctor identify any conditions that could suggest you should not take ADHD medication if prescribed.

If you are being titrated, the medication will consist of a mixture of stimulants and non-stimulant drugs. These will be carefully trialled and monitored to find the most effective combination for you, taking into account any medical conditions you may have. The titration process can be lengthy and may need several additional appointments before you are stable on your dose.

The frequency of your follow-up appointments will be contingent on the type and amount of medication that you are taking. They can be monthly or quarterly. Your doctor will ensure that you are stable with the medication, record any side effects you have experienced and make any necessary adjustments to ensure that you remain on a safe dosage. You will be required to attend an annual review and a regular health check-up that includes pulse, blood pressure and weight measurements.

Some people might be satisfied with their treatment and do not have to discuss their titration further. If, however, they require further discussion about their titration and treatment plan it's possible that they will be referred back to the private clinic that conducted their initial assessment. If this occurs, the titration consultation will be part of their consultation fee, and they will be given their medication on an individual prescription (see Consultation fees above). This can cost more than prescriptions from the NHS, as the pharmacy will charge for the medication.
